The Population Health Coordinator will be responsible for the overall strategy and coordination of population health initiatives and quality payment programs. The incumbent will collaborate within the Hospital’s provider network to improve quality, efficiency, and cost of healthcare services on behalf of its patient population. The incumbent will be responsible for researching quality payment programs across the Hospital’s payer network and collaborating with the Hospital’s quality and process improvement teams to maximize the quality and financial benefit to both the patient population and the Hospital. The incumbent will be required to collaborate with the Information Technology department, Revenue Cycle departments, Case Management department, providers and clinicians to train and educate all stakeholders to improve all programs identified. The incumbent will serve as the Hospital’s primary ACO leader and champion.

Education

Requirements

Incumbent must be a graduate of school of nursing. Bachelor’s Degree is preferred.

Experience

Requirements

Effective interpersonal communication skills in order to effectively collaborate with physicians and instruct patients and their families. Three years of relevant management experience with knowledge of:

Population health programs, including annual wellness visits, chronic care management, and similar programs

Accountable care organizations (“ACO”) and impact of Medicare Shared Savings Programs

Quality payment programs from various other third-party payers

Certifications/ Licensures

Requirements

Current West Virginia license. Must possess BLS certification within probationary period. Must obtain Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support within one year.

Light Work: Frequent lifting and/or carrying of objects weighing up to 10 lbs. with a maximum lifting of 25 lbs.

Schedule: Full Time, Flex
