Population Health Care Coach- Medical Assistant/LPN

As a member of the health care team, assists with population health management through patient outreach and coordination of care, participates in the clinical visit to assist the provider, and provides self-management support to patients. Demonstrates leadership by communicating effectively, developing others, building and maintaining relationships, and guiding effective teams and work groups. Also mentors others within the team with office functions to support patient care and office operations. Follows all established protocols, policies, procedures and standardized work flows. Leads in managing data so that patients are effectively treated.
MINIMUM QUALIFICATIONS
Education:
A graduate of a medical assisting or LPN program
Experience:
Two Years Medical Assistant or LPN Experience
Licensure/Certification:
CPR within Probationary period
Location: Mon Health Medical Center ยท Population Health
Schedule: Full Time, Days, 7.30 AM - 4 pm with half hour lunch break
As a condition of employment and in compliance with federal regulations, Mon Health requires COVID-19 vaccination.
